phpMyAdmin is the right place for that.

When you log in to phpMyAdmin there is a text box where you simply type the name of the new database and press the button "Create".

Your host may already have created a database for you, and you may not have rights to create more databases. Then you just use the assigned database and create tables in it. To create a table, click on your database. There will be a box titled "Create new table in database" where you type the name of the table and the number of columns and press the button "Create". Then you can name columns and set the data type, etc.

Hope it make sense. Otherwise, a new PHP tutorial will soon be online here at HTML.net :)
